Important Patient Information

As a new patient, we will need you to complete all new patient forms (listed adjacent – please click to access forms you need) and please return them as soon as possible before your appointment. Include all pertinent medical information, including MRI, X-rays, blood tests, and a list of all medications.  




To make a first time appointment, please call us at (707) 546-7979. You may need a referral. We will be happy assist you in contacting your primary care physician if needed.

We also have staff members dedicated to coordinating patient care with insurance carriers, adjusters, nurse case managers and employers. Our staff mission is to provide:

Prescriptions:

We endeavor to provide timely prescription refills. Please call 48 hours in advance and during normal business hours and have the prescription and pharmacy name and phone number available for our staff.

We respectfully request that after normal hours phone calls be emergency related to allow for our physicians to focus on providing timely and quality care to our patients.

We ask that our patients play an active role in monitoring the status of their prescriptions and plan ahead for weekends and holidays. We ask that our patients be understanding with regards to our physicians who may be in surgery and would not receive patient requests until the following day.

Health Insurance and Payment:

We accept all Insurances including: Private insurance, Blue Cross / Blue Shield, Medicare, and Workers’ Compensation, MPN, OWCP, Tricare, Dept of Labor, Partnership Health Plan.

Personal Injury / Motor Vehicle Accidents:
We also accept liens for personal injuries and motor vehicle accidents.  We will work with patient attorneys to expedite and resolve their cases!

Workers’ Compensation

APMG prides itself in providing injured workers with timely and high quality care. We also believe in working with insurances to provide them with timely information and treatments.
